Course Description:  Research has shown six overlapping categories of problems have been associated with negative outcomes among hospitalized older adults with multiple chronic conditions who transition to post-acute settings or their homes: lack of patient engagement; absent or inadequate communication; lack of collaboration among team members; limited follow-up and monitoring; poor continuity of care; and, serious gaps in services as patients move between healthcare professionals (clinicians) and across care settings. This course will assist healthcare workers in planning effective care planning for patients.

Target Audience:  Nursing, Social Workers, Counselors and Therapists

Learning Outcomes:

At the end of this presentation, the learner will be able to:

  • describe the importance of completing assessments.
  • discuss motivational interviewing.
  • describe their role in the assessment and care planning process.

Credit Statements:

 Licensed Nurses:  Upstate AHEC is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.

Upon successful completion of this program, attendees will receive 3 contact hours.

Counselors and Therapists: This program has been approved for 3 hours of continuing education by SC AHEC under its accreditation by the SC Board of Examiners for Licensure of Professional Counselors, Marriage and Family Therapists and Psycho-Educational Specialists.
