Course Description: This course will provide participants with skills and knowledge to have a conversation about Advance Directives and End of Life Care.

Target Audience: All Healthcare professionals
- Define Advance Directives and the benefits of timely completion of these documents.
- Describe the two types of Advance Directives: Healthcare Power of Attorney and Living Will.
- Describe how to have a conversation with loved ones about Advance Directives, personal wishes and how to manage this document.
- Identify different resources to assist in development of Advance Directive.
Length of Course: 90 minutes
Licensed Nurses: Upstate AHEC is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.
Upon successful completion of this program, participants will receive 1.50 contact hour.
