Course Description:  As the landscape of health care continues to evolve at a rapid pace, we must be sure that we are applying the evidence that helps support patient-centered care. Health care organizations are now in the transition from fee based initiatives to value based care. Data and evidence are not part of our daily care routines and we can use these tools to help us achieve the Triple Aim of Health Care: Improving the patient experience of care (including quality and satisfaction); Improving the health of populations; and Reducing the per capita cost of health care.

Target Audience: Nurses and Respiratory Care Practitioners and other interested healthcare providers

Learning Objectives: At the end of this presentation the learner will be able to:

  • State prominent reasons for frequent readmissions
  • Identify social disparities that contribute to frequent readmissions
  • Explain concepts to reducing readmissions
  • Formulate a strategy to reduce readmissions and prevent admissions
